The Community Health Trust is a broad, self-sustaining partnership of community organizations serving northwestern New Jersey.

Through new and innovative approaches to health, prevention and education, we seek to unify the community, stimulate collaborative action and link people with resources to create a healthier future.
 
 

SHARED VISION

The Community Health Trust, a community partnership sponsored by Saint Clare's Health Services, brings people together in a spirit of partnership that unites individuals, neighborhoods and towns toward a common goal: a healthier community.

These partnerships help create a seamless network of resources that ensure continuous improvement in health and ongoing involvement in programs that benefit the well-being of the entire community.

 The Trust's Board of Partners determines policy, develops strategic planning, monitors fiscal areas and oversees these four committees: Community Action Teams, the Measurement and Evaluation Team, the Membership and Public Relations Team, and the Fund Development Team. Committee responsibilities range from measuring program outcomes and membership recruitment to fundraising and action team participation. If you, your organization or business shares our vision for a community that is healthier, motivated to work for the well-being of its citizens and more intent on solutions than problems, we invite you to become an active partner of the Community Health Trust.
